Comparison of SARS-CoV-2 Detection from Nasopharyngeal Swab Samples by the Roche cobas® 6800 SARS-CoV-2 Test and a Laboratory-Developed Real-Time RT-PCR test.

  • The urgent need to implement and rapidly expand testing for Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) infection has led to development of multiple assays. How these tests perform relative to one another is poorly understood. We evaluated the concordance between the Roche Diagnostics cobas® 6800 SARS-CoV-2 Test and a laboratory- developed Real-Time RT-PCR test (LDT) based on a modified Centers for Disease Control and Prevention (CDC) protocol, for the detection of SARS-CoV-2 in samples submitted to the Clinical Laboratories of the Mount Sinai Health System. 1006 Nasopharyngeal swabs in universal transport medium from persons under investigation were tested for SARS-CoV-2 as part of routine clinical care using the cobas SARS-CoV2 test with subsequent evaluation by the LDT.
  • Cycle threshold values were analyzed and interpreted as either positive (‘Detected’ or ‘Presumptive Positive’), negative (‘Not Detected’), inconclusive, or invalid. Statistical analysis was performed using GraphPad Prism 8. The cobas SARS-CoV2 test reported 706 positives and 300 negatives. The LDT reported 640 positives, 323 negatives, 34 inconclusive, and 9 invalids. When excluding inconclusive and invalid results, the overall percent agreement between the two platforms was 95.8%.
  • Cohen’s kappa coefficient (κ) was 0.904 (95 % CI 0.875-0.933), suggesting almost perfect agreement between both platforms. An overall discordance rate of 4.2% between the two systems may reflect differences in primer sequences, assay limit of detection, or other factors, highlighting the importance of comparing the performance of different testing platforms. The accuracy of the Montenegro skin test for leishmaniasis in PCR-negative patients.

As highly specific molecular biology-based techniques may not be sensitive enough for the diagnosis of American tegumentary leishmaniasis (ATL), clinicians frequently rely on immunological tests before treatment initiation. Hence, the correct combination of diagnostic tests is imperative for ATL diagnosis. We aimed to evaluate the accuracy of the Montenegro (Leishmanin) skin test (MST) in polymerase chain reaction (PCR)-negative patients to accurately detect ATL.Patients with a clinical picture compatible with ATL were divided into ATL (confirmed by lesion smear, culture indirect immunofluorescence, and/or histopathology) and no-ATL (diseases that can mimic leishmaniasis) groups. Conventional PCR for the minicircle kDNA of Leishmania was performed, and the MST was carried out for PCR-negative patients.Ninety-nine patients were included in this study, including 79 diagnosed with ATL (6 with mucocutaneous leishmaniasis) and 20 without ATL (no-ATL group).
The MST showed a high sensitivity of 90.0% (95% confidence interval [CI] = 69.90-97.21) in PCR-negative patients that was 10% higher than the sensitivity reported in PCR-positive population (79.66%; 95% CI = 67.73-87.96).One of the most important reasons for PCR negativity among patients with active ATL is the presence of a strong cellular immunological response, especially in chronic and mucocutaneous leishmaniasis. This reinforces the considerable utility of the tests that detect cellular responses against Leishmania antigens such as the MST in PCR-negative patients when the performance in screening situations is questionable.

Positive RT-PCRtests among discharged COVID-19 patients in Shenzhen, China.

Among all 209 discharged coronavirus 2019 patients in Shenzhen China between January 23 and February 21, 2020, there are 9 (4.3%) patients showed RT-PCR positive in throat swabs, 13 (6.2%) patients showed RT-PCR positive in anal swabs, and 22 (10.5%) positive in either type.
The time between discharge and positive RT-PCR tests is 4.7 days on average. These numbers shed light on the viral dynamics of COVID-19.

A case report of COVID-19 with false negative RT-PCR test: necessity of chest CT.

The definite diagnosis of corona virus disease 2019 (COVID-19) is based on the viral isolation or positive result of polymerase chain reaction (PCR) from sputum, or nasal swab, or throat swab. However, the sensitivity to detect COVID-19 of real time (RT)-PCR is reported to be lower than that of chest CT. We report a case of 34-year-old man who was diagnosed as negative for COVID-19 based on the four sequential RT-PCR tests of his pharyngeal swab.
Chest CT showed patchy ground-glass opacity on admission, and it rapidly progressed to segmental mixed consolidation and ground-glass opacity 3 days after admission, and it resolved in left upper lobe, but showed multifocal ground-glass opacities 7 days after admission, and they resolved within 2 weeks. The fifth RT-PCR test finally revealed positive results at the fifth day after admission.
It is difficult to distinguish COVID-19 pneumonia from other viral pneumonia on CT findings alone; however, we emphasize the utility of chest CT to detect early change of COVID-19 in cases which RT-PCR tests show negative results
