
stablishment of a novel diagnostic test for Bovine leukemia virus infection using direct filter PCR.
- Enzootic bovine leukosis (EBL) is a neoplastic disease of cattle caused by Bovine leukemia virus (BLV). EBL causes great economic losses, so a fast and reliable diagnostic method is critical for understanding the status of BLV. This will allow us to control BLV infections efficiently and mitigate economic losses.
- In this study, we established a direct diagnostic test for BLV using dried blood-spotted filter papers without sample pretreatment. The study was based on 159 clinical blood specimens collected in EDTA from one farm in Kyushu, Japan. The blood-spotted filter papers were used as the template for direct filter PCR. When an ELISA was used as the diagnostic gold standard, the sensitivity and specificity of the direct filter PCR were 90.1% and 97.5%, respectively.
- The kappa value for the direct filter PCR and real-time PCR methods was 0.97. The dried blood samples spotted onto filter papers were stable for at least 10 days at room temperature, even when the samples were from cattle with a low BLV proviral load. Direct filter PCR is a rapid, easy, reliable, and cost-effective diagnostic test that directly detects the BLV proviral genome in clinical blood specimens without DNA extraction. Moreover, it simplifies the collection, transportation, and storage procedures for clinical blood specimens.

Multicenter Evaluation of the Implementation Status of Laboratory Tests in Korea and the Potential Usefulness of a Multiplex PCR Assay in Patients with Suspected Central Nervous System Infections.
- The rapid diagnosis and treatment of central nervous system (CNS) infections are critical to minimizing morbidity and mortality. We evaluated the implementation status of laboratory tests in patients with suspected CNS infection, and the potential usefulness of a multiplex PCR assay for rapid and simultaneous detection in cerebrospinal fluid (CSF) of 14 targets capable of causing CNS infections.The study was conducted at 5 hospitals located in Daegu and Gyeongju over a period of 6 months.
- A total of 140 patients with suspected CNS infection were included. CSF samples were tested for 6 bacteria, 7 viruses, and 1 yeast using multiplex PCR (FilmArray Meningitis/Encephalitis Panel, BioFire Diagnostics/Biomerieux, Salt Lake City, UT, USA) and conventional diagnostic testing including chemistry tests, cell count, bacterial culture, antigen detection assay, and pathogen-specific PCR.The five conventional tests most commonly performed were the chemistry and cell count (100%), bacterial culture (94.3%), enterovirus PCR (52.9%), and herpes simplex virus PCR (25.7%). Among the 140 CSF samples, 27 (19.3%) and 42 (30.0%) tested positive by conventional and the FilmArray ME panel testing, respectively.
- The detection rate of pathogens for CNS infections increased using only one FilmArray test compared to all of the conventional methods actually performed in patients with suspected CNS infection.
- Diagnostic accuracy of a non-invasive test for the detection of Helicobacter pylori and resistance to clarithromycin in stool by Real-Time PCR Amplidiag®H. pylori + ClariR assay.
- Non-invasive detection of Helicobacter pylori and its resistance to clarithromycin could revolutionize the management of H. pylori-infected patients by tailoring eradication treatment without any need for endoscopy when histology is not necessary. If several real-time PCR tests performed on stools have been proposed, their performances were either poor or tested on too few patients to be properly evaluated.
- We have conducted a prospective, multicenter study including 1200 adult patients addressed for gastro-duodenal endoscopy with gastric biopsies and naive of eradication treatment in order to evaluate performances of the Amplidiag®H. pylori+ClariR assay recently developed by Mobidiag (Espoo, Finland).
- The results of the Amplidiag®H. pylori+ClariR assay performed on DNA stools (automatic extraction, EasyMag bioMérieux) were compared with those of the culture/E-test and quadriplex real-time PCRs performed on two gastric biopsies (antral and corpus) so as to detect H. pylori glmM gene and mutations in the 23S rRNA genes conferring clarithromycin resistance. Sensitivity and specificity of the detection of H. pylori were 96.3% (CI 92-98) and 98.7% (CI 97-99) respectively. Positive and Negative Predictive Value were evaluated at 92.2% (CI 92-98) and 99.3% (CI 98-99) respectively. In this cohort, 160 patients (14.7%) were found to be infected (positive in culture and/or PCR).
- Sensitivity and specificity for detecting resistance to clarithromycin were 100% (CI 88-100) and 98.4% (94-99), respectively.
